ENROLLMENT FORM

An enrollment form must be completed for each school bus driver attending a training session. Registration fee is $75.00 for each driver. Failure to cancel within two days of the first class date will result in a forfeiture of the fee. Purchase orders and/or checks should be addressed and made payable to Wayne RESA, 33500 Van Born Rd., Wayne, MI 48184. Should you have any questions please call the Transportation Office at (734) 334 -1314.

Training will be held at Wayne RESA. Training is scheduled from 7:00 a.m. until 1:00 p.m. on days one through three, and 10:00 a.m. until 1:00 p.m. on day four. Attendance is required for all four days. You will be sent a confirmation letter with the Certificate of Enrollment card.

By the electronic submission of this form to Wayne RESA, the Pupil
Transportation Supervisor has certified that the above named school bus driver
has met the Michigan Department of Education's school bus driver prerequisite
"Behind the Wheel" training program.
