Caring 4 Students

In order to bill for C4S, students must:

Be Medicaid eligible
Under the age of 21
Have a plan of care in place (i.e. 504 plan, behavior plan, nursing plan, individualized health plan, etc.)
Have a direct, direct/consult service on their plan of care with frequency and duration
One-Time Parental Consent to bill Medicaid
